Free billing reference
The FHO+ codes most practices forget to bill
Charting, results review, referrals, care planning. You already do the work. FHO+ has codes for it, and they are the easiest ones to miss.
The quick reference puts the hourly codes, the after-hours and weekend conditions, the roster workflow and forty-plus billing codes in one place you can keep by the desk.
Who it is for
- Family physicians in an FHO or FHO+ group who suspect they are under-claiming and have no time to read the Schedule of Benefits to find out.
- Clinic managers and billing staff who need one agreed reference instead of four conflicting ones.
- Residents and physicians new to the model who were taught medicine, not billing.
What is on the sheet
- The two hourly codesQ310A and Q312A, and the line between direct and indirect patient care that decides which one applies.
- Caps and the rounding ruleDaily and monthly maximums, and the 15-minute rule that quietly changes your unit count.
- After-hours and weekend premiumsQ012A and Q888A, and the conditions that have to line up before either is payable.
- The meet and greet workflowRoster billing, which runs differently for patients over and under 30.
- Unattached patientsThe codes that must also go to the Ministry for Health Care Connect referrals.
- The code libraryUnits per 12 months, In Basket or Out of Basket, and the exceptions that qualify them.
Where it came from
We built this for our own practice. Mimico Wellness Management runs family medicine clinics in Etobicoke, and this is the sheet our billing runs on day to day.
It is consolidated from Ontario Ministry of Health FHO+ INFOBulletins 260308, 260309 and 260501, the current Schedule of Benefits for Physician Services, and Ministry after-hours and weekend-access guidance. Dollar values are deliberately left out, because fee values change and a stale number is worse than none.

A quick operational reference, not billing advice. It does not replace the current OHIP Schedule of Benefits, your FHO agreement, or Ministry and OMA billing updates. A code appearing on the sheet does not mean it is payable in every encounter. Apply the Schedule rules, limits and model-specific conditions, and confirm current fee values before submission.
